Why air filtration matters during amalgam removal
During amalgam removal, mercury vapor is released into the treatment room air regardless of how carefully the procedure is conducted. High-volume evacuation (HVE) captures the majority of vapor at the source, but a portion escapes into the ambient room air β where it can be inhaled by both the patient and the dental team throughout and after the procedure.
Room-level air filtration is the third line of defense in SMART protocol (after rubber dam isolation and HVE). Its job is to remove airborne mercury vapor and fine particulate from the treatment room during and after removal procedures.
Types of filtration systems
There are three main types of filtration relevant to mercury vapor in dental settings:
- HEPA filtration: High-Efficiency Particulate Air filters capture particles β₯0.3 microns at 99.97% efficiency. They capture fine amalgam particulate and larger aerosols but do not capture mercury vapor (a gas, not a particle).
- Activated carbon filtration: Carbon (charcoal) filters adsorb mercury vapor and many volatile organic compounds at a molecular level. This is what actually captures gaseous mercury β HEPA alone doesn't do this.
- Standard HVAC: Building-level heating and ventilation systems are not designed to filter mercury vapor and are completely inadequate for this purpose.

What HEPA actually means
HEPA is a performance standard, not a brand. A filter qualifies as HEPA if it captures at least 99.97% of particles 0.3 microns or larger. This is effective for amalgam particulate, aerosols, and other fine debris released during drilling.
However, elemental mercury vapor molecules are much smaller than 0.3 microns and pass straight through HEPA filters. This is why activated carbon β which works through chemical adsorption rather than mechanical filtration β is required as a companion filter for genuine mercury vapor control.
Some lower-cost air purifiers marketed to dental offices use HEPA alone and do not include activated carbon. When evaluating a provider, ask specifically: "Does your air filtration system include activated carbon for mercury vapor, or only HEPA?"

A dentist who uses a dedicated HEPA + activated carbon unit in the treatment room, with current filters, is meeting the air filtration component of SMART protocol. A dentist who relies on building HVAC is not.
