How to think about this decision
This is one of the most genuinely personal decisions in dentistry. Unlike choosing which filling material to use (where the dentist has clinical input that matters), the question of whether to remove stable, intact amalgam fillings is largely driven by patient values, risk tolerance, and individual health context.
There is no universal "right answer." Patients who leave stable fillings in place are making a defensible choice. Patients who choose removal are also making a defensible choice. What matters is that the decision is made with clear eyes about what the evidence does and doesn't show.
The honest case for removal
- Eliminates ongoing low-level exposure. Amalgam fillings do release small amounts of mercury vapor continuously, primarily during chewing. Removal eliminates that source permanently.
- Precautionary logic. For patients who are pregnant, planning pregnancy, have kidney disease, or have a known mercury sensitivity, the risk-benefit calculation may favor removal.
- Filling age and condition. Old amalgam can corrode, crack, or develop marginal leakage. Replacing aging fillings with modern materials before they fail is clinically reasonable, and you can choose SMART protocol for the removal.
- Personal peace of mind. For some patients, the psychological benefit of removal is real and valid. This is a legitimate factor in a personal health decision.

Clinical reasons that change the math
While elective removal of stable fillings is a genuinely personal decision, there are clinical situations where removal is less ambiguous:
- The filling is cracked, fractured, or leaking β creates both decay risk and possible mercury release
- There is recurrent decay under or around the filling
- The filling is so old it is likely to fail in the near term anyway
- The tooth has a crack that requires a crown regardless
- You are pregnant or planning to become pregnant and want removal done before conception

How to actually decide
Have an honest conversation with a qualified dentist who will give you a straight answer rather than an agenda-driven one.
