Indirect and logistical costs
- Time off work: A typical SMART removal appointment runs 1.5–2.5 hours per session. Add travel time, and a half-day off work is standard for most patients. If you're having multiple sessions, multiply accordingly. For salaried employees this may be absorbed as PTO; for hourly or self-employed patients it's a real financial cost.
- Childcare: Many patients don't think about this until the day of the appointment. If you have young children, budget for childcare during each session.
- Transportation: If you're choosing sedation, you cannot drive home. You'll need someone to pick you up or arrange a rideshare — and they may need to wait at the office for the duration of your appointment.
- Parking and transit: In urban areas, parking at or near a dental practice can cost $15–$40 per visit. Across multiple appointments this adds up.
- Meals: After composite placement you'll be instructed to avoid certain foods. Many patients end up buying softer or more specific foods during recovery days.
Follow-up and revision costs
- Bite adjustment visits: Post-placement bite issues are common and typically require a quick follow-up visit. Most practices don't charge for this within 30–60 days of placement, but confirm in advance.
- Sensitivity treatment: Some patients experience significant post-operative sensitivity that requires follow-up care or desensitizing treatment. Usually not an additional charge if within normal warranty period, but ask about the practice's policy.
- Failed restorations: Composites occasionally fail — debonding, chipping, or cracking. Most practices warranty their work for 1–2 years; beyond that, replacements cost full price. Ceramic restorations may be under manufacturer warranty but labor is not.
- Unexpected complexity: Once a large amalgam filling is removed, the dentist may find more decay underneath, a cracked tooth, or insufficient remaining structure. This can turn a planned composite into a more expensive crown. Ask your dentist to estimate the likelihood of this scenario for your specific fillings before you begin.
Supplement and detox protocols
Many biological dental practices recommend pre- and post-procedure nutritional protocols, typically involving chlorella, vitamin C, glutathione, or other supplements intended to support mercury clearance. This is not part of the formal SMART protocol and the evidence base is limited.
If a practice sells these protocols in-office or strongly recommends specific brands, budget $100–$400 for supplements. If you're paying for IV nutritional therapy as part of a removal package, costs can run $200–$800 per session. These costs are never covered by insurance and are not clinically required for a successful SMART procedure.
Travel costs for specialists
SMART-certified dentists are not evenly distributed. In smaller cities or rural areas, patients sometimes travel 2–4 hours to reach a qualified provider. If that's your situation, factor in:
- Round-trip mileage or flights
- Hotel if multiple sessions are scheduled on back-to-back days (which many practices prefer for out-of-town patients)
- Meals during travel days
- Two days of time off work minimum per trip
For patients who need 4–6 fillings removed, a two-session approach requiring one overnight trip might add $500–$1,000 in travel costs on top of treatment fees. Some patients find it more cost-effective to do all fillings in a single extended visit rather than multiple trips.
